Schedule of Field Trips for Summer of 2013

Field Trip Schedule. Be sure to fill out a permission form for each child.

Print this form and fill out for each child attending sports camp. Bring to the church office with payment.

Fill in this form for the third or successive camper when there are multiple campers in the family.

Fill in this form for the second camper when there are multiple campers in the family.

If you have MORE THAN ONE CHILD scheduling for day camp, use this form for the first camper in the family.

If you are scheduling ONE CHILD for summer day camp, use this form.  Download this form, print, complete, and bring to the church office.
